Because of the dynamic modules used in preparing this form, some screen readers do not always read the fields correctly. If you are using Jaws for Windows, version 7.0 or earlier, follow these steps to ensure that the field labels will read properly as you tab through the form.
1. Start Internet Explorer, the page you are on does not matter.
2. Press the Jaws key, usually insert, and 6 on the numeric row, not the keypad. This will take you into the Jaws Configuration Manager.
3. Press the alt key and s to get to the set options menu.

4. Down arrow until you hear HTML options, one or two presses, and press enter.
5. Press shift and tab together to get to the series of tabs for different option screens.
6. Right arrow until you hear the "Misc" tag.
7. tab until you hear a check box that says "Use virtual labels in forms mode." NOTE: This is not present in version 8. This will very probably be unchecked.
8. Press the space bar to check it.
9. Tab until you hear OK and press enter.
10. Press control and s together to save the changes.
11. Press Alt f, then x to get out of the configuration manager.
